Committee Request Form

From time to time, all committees and sub-ministries at Phoenix URC need support. You might need more chairs, extra volunteers, or just some advice on how to improve your area of service. Whether the need is material, personal, or spiritual, we would like to help.

This form goes to our Associate Pastor. He will confirm with you and make sure it gets to the appropriate person(s) to ensure you receive assistance in a timely fashion.
